• PV is a one school district and we have one CST for that building, consisting of a Learning Specialist (LDTC), School Psychologist and School Social Worker. The team ‘coordinates the development, monitoring and evaluation of the effectiveness of the IEP (Individualized Educational) Plan.’ The CST Case Manager ‘facilitates communication between home and school and shall coordinate the annual review process. The case manager shall be knowledgeable about the student’s educational needs and program, shall be knowledgeable about special education procedures …have an apportioned amount of time for case management… and be responsible for transition planning’ (6A: 14-3.2). Transition planning consists of post high school college and career planning and if appropriate, independent living.
